How it works

AUDIT = Q1–Q8 scored 0–4 + Q9 and Q10 scored 0, 2 or 4; range 0–40. WHO zones: I 0–7 · II 8–15 · III 16–19 · IV 20–40. AUDIT-C = Q1 + Q2 + Q3 (0–12).

The Alcohol Use Disorders Identification Test was developed by the World Health Organization to detect hazardous and harmful drinking and possible dependence in primary care. Questions 1–3 cover consumption, 4–6 dependence symptoms and 7–10 alcohol-related harm. The WHO manual (Babor et al., 2001) links each score range to a level of intervention, from education through brief advice to specialist referral. Points on questions 4–6 or on questions 9–10 deserve attention even when the total is modest.

Worked example

Drinking 2–3 times a week (3), 3 or 4 drinks on a typical day (1), six or more drinks monthly (2), never unable to stop (0), failing expectations less than monthly (1), no morning drinking (0), guilt less than monthly (1), a blackout less than monthly (1), no injuries (0) and a doctor who was concerned, but not in the last year (2): AUDIT = 11 / 40 — Zone II, hazardous drinking, for which simple advice is recommended. The AUDIT-C part (Q1–Q3) is 6 / 12.

Frequently asked questions

What is a high AUDIT score?

Scores of 8 or more indicate hazardous or harmful drinking in the WHO manual. Scores of 20 or more (Zone IV) suggest possible alcohol dependence and warrant specialist assessment. Some guidelines use 7 for women and people over 65.

What is the AUDIT-C?

The first three questions on their own (0–12). A score of 4 or more in men or 3 or more in women is a common positive screen for unhealthy drinking.

How many grams is one drink?

The WHO AUDIT assumes a standard drink of 10 g of pure alcohol. Countries with larger standard drinks (14 g in the US) sometimes adapt question 2.

Is the AUDIT a diagnosis?

No. It is a screening test. A diagnosis of alcohol use disorder needs a clinical assessment.
