How it works

Sum of seven items scored 0–3 over the last two weeks (0–21); 0–4 minimal, 5–9 mild, 10–14 moderate, 15–21 severe; ≥ 10 = probable anxiety disorder

The GAD-7 was developed alongside the PHQ-9 as a brief, free measure of generalised anxiety that also picks up panic disorder, social anxiety and post-traumatic stress reasonably well, making it the default anxiety screen in primary care and IAPT-style services. Scores of 10 or more identify a probable anxiety disorder that deserves a proper assessment; 15 or more indicates severe symptoms. Like the PHQ-9 it doubles as a progress measure — a drop of 4 points or more is considered a reliable improvement, and below 8 is the usual recovery threshold in UK talking-therapy services. It does not assess suicidality, so pair it with the PHQ-9 when depression may coexist, which is common.

Worked example

More than half the days for nervousness, uncontrollable worry and worrying about many things (6), several days for trouble relaxing, irritability and dread (3), never restless: total 9 — mild; GAD-2 = 4, screen positive, so a clinical review is reasonable.

Frequently asked questions

What is a clinically meaningful change?

A reduction of 4 or more points, or moving below the threshold of 8 (UK) / 10 (US), indicates reliable improvement.

Can adolescents use it?

It has been validated from about age 12–13 upward; for younger children use SCARED or RCADS instead.
