SCORAD Calculator — Atopic Dermatitis (Eczema) Severity
Calculate SCORAD for atopic dermatitis (eczema): extent of affected skin, intensity of six signs and itch and sleeplessness scores — total 0–103 and objective SCORAD 0–83 with mild, moderate or severe.
How it works
SCORAD = A/5 + 7B/2 + C, where A = extent (% body surface, 0–100), B = sum of six intensity items (erythema, oedema/papulation, oozing/crusts, excoriation, lichenification, dryness; each 0–3, total 0–18), C = itch + sleeplessness (0–10 each). Objective SCORAD = A/5 + 7B/2 (0–83). Severity: SCORAD < 25 mild, 25–50 moderate, > 50 severe; oSCORAD < 15 mild, 15–40 moderate, > 40 severe.
SCORAD (SCORing Atopic Dermatitis) was developed by the European Task Force on Atopic Dermatitis (Dermatology 1993) and is one of the two core instruments, with EASI, for eczema trials. Intensity items are graded at a representative area of average intensity, not the worst spot, and itch and sleeplessness are rated by the patient or parent on a 0–10 scale for the last three days and nights. The objective SCORAD drops the subjective symptoms for comparisons between observers.
Worked example
Affected area 20% gives A/5 = 4.0. Intensity scores erythema 2, oedema 2, oozing 1, excoriation 2, lichenification 1 and dryness 2 add to B = 10, so 7B/2 = 35.0. Itch 6 + sleeplessness 4 gives C = 10. SCORAD = 4 + 35 + 10 = 49.0 / 103 — moderate; objective SCORAD 39.0 (moderate).

Frequently asked questions
What SCORAD score is severe eczema?
A SCORAD above 50 is severe, 25–50 moderate and below 25 mild (European Task Force). For the objective SCORAD the cut-offs are 15 and 40.
How do I estimate the affected area?
Use the rule of nines: head and neck 9%, each arm 9%, front and back of the trunk 18% each, each leg 18% and genitals 1%; in children under 2 the head is 17% and each leg 12%. The palm of the patient’s hand with fingers is about 1%.
What is the difference between SCORAD and EASI?
Both measure extent and intensity, but SCORAD adds patient-reported itch and sleep loss and scores dryness, while EASI uses four regions and four signs without symptoms.
What is a meaningful SCORAD improvement?
Trials often report SCORAD 50 or 75 (a 50% or 75% reduction). A change of roughly 8–9 points is a commonly cited minimal clinically important difference.
