How it works

Each system 0–4: Resp (P/F ≥ 400 / < 400 / < 300 / < 200 vent / < 100 vent), Coag (plt ≥ 150 / < 150 / < 100 / < 50 / < 20), Liver (bili < 20 / 20–32 / 33–101 / 102–204 / > 204 µmol/L), CV (MAP / pressor dose), CNS (GCS 15 / 13–14 / 10–12 / 6–9 / < 6), Renal (Cr < 110 / 110–170 / 171–299 / 300–440 or UO < 500 / > 440 or UO < 200)

The Sequential Organ Failure Assessment grades dysfunction in six organ systems from 0 (normal) to 4 (severe failure) and was designed to be tracked daily in the ICU. Since Sepsis-3 it also defines sepsis: a rise of two or more points from baseline in a patient with suspected infection (assume a baseline of zero if the patient was previously well). Total SOFA correlates with mortality — roughly 10% below 4 and above 80% at 14 or more — and rising scores over the first 48 hours predict death better than any single measurement. The respiratory component depends on whether the patient is ventilated, and the renal score takes the worse of creatinine and urine output. Sedation confounds the neurological score; use the best estimate of GCS off sedation.

Worked example

P/F 250 on oxygen (2), platelets 90 (2), bilirubin 40 (2), dobutamine (2), GCS 13 (1), creatinine 180 (2): SOFA 11 — a rise of 11 from a healthy baseline, mortality around 50%.

Frequently asked questions

How do I score a patient on high-flow oxygen without an arterial gas?

Estimate PaO₂ from SpO₂ (SpO₂/FiO₂ ratio ≈ 64 + 0.84 × PaO₂/FiO₂) or use the S/F substitutes: S/F < 315 ≈ P/F < 300, S/F < 235 ≈ P/F < 200.

What is the difference between SOFA and qSOFA?

qSOFA is a three-item bedside prompt outside ICU; SOFA needs lab values and quantifies actual organ dysfunction — it is what defines sepsis.
